No matter which coverage option you choose, you'll only have a $100 deductible, which is pretty reasonable compared to many other dental insurance policies. I don't know how I got roped into this policy... after $400 in premiums, I was papered to death to collect $100 in benefits, and found even when I shopped at Costco they say the cost is above what's allowed. You have to fill out multiple forms with your doctor, go through reviews, have a deductible of $100/year, and a wait period of 6 months to claim anything. I did a spreadsheet. The yearly limit to benefits means you would have to claim the full amount for each part of the plan (dental, vision, hearing) each year and get full benefits for over 7 years to even break even.
